Employee crushed in tip-over incident

Employee #1 was servicing a Lull Reachout Rough Terrain Forklift (RTFL). The RTFL which weighed 23,160 lbs. It was parked on an incline and tipped over when the mechanic raised the boom to check the problem. The boom was raised from a boom angle of 0 degrees to a boom angle of approximately 62 degrees. The stability of the RTFL was compromised when the combination of the raising of the boom and the incline in which it was parked caused the tipping action when the center of gravity shifted outside of the stability triangle. Once the boom was raised in its location, the RTFL became unstable and exceeded the tolerance of the RTFL to remain on its four wheels. When the RTFL tipped over, Employee #1 was crushed to death beneath it.
