Investigation Summary
EMPLOYEE #1 (A SUPERINTENDENT) WAS STANDING BESIDE A TRACK PROPELLED BACKHOE, DIRECTING A PIPE-LAYING OPERATION. THE BACKHOE WAS BEING USED TO LIFT 55 FOOT LENGTHS OF 15 INCH DIAMETER STEEL CASING, SWING THE CASING OVER THE TRENCH AND LOWER IT INTO PLACE. AS A LENGTH OF CASING WAS BEING MOVED TOWARD THE TRENCH, ONE END CAUGHT IN THE BANK OF THE DITCH. EMPLOYEE #1 FREED THE STUCK END BY PULLING ON A ROPE TIED TO IT. WHEN THE CASING WAS FREED, ONE END SWUNG ACROSS THE DITCH AND STRUCK EMPLOYEE #1 IN THE HEAD. HE WAS KILLED WHEN HIS HEAD WAS CRUSHED BETWEEN THE PIPE AND THE BACKHOE CAB.
