Investigation Summary
On October 29, 2001, Employee #1 was in a 5-ft-deep by 4-ft to 5-ft-wide by 55-ft to 60-ft-long trench, looking for the main sewer line to start a hookup. He was stooped over when one wall of the trench caved in. He sustained injuries that required hospitalization. The wall that fell in on Employee #1 ran parallel with a gas line trench that had been dug two years earlier and that was only 2 ft away from the collapsed wall. The proximity of the two trenches made the near wall of Employee #1's trench unstable and led to its collapse.
