Employee Crushed And Killed By Forklift

Employee #1 was operating a forklift truck with a box clamp attachment to place a load in storage. The mast was fully extended to approximately 13 ft. When Employee #1 backed away, he made a sharp turn that caused the forklift to overturn. The forklift was equipped with seat belts but the operator was not wearing them. Employee #1 suffered lacerations and was killed.
