Investigation Summary
On August 21, 2001, Employee #1 and his son were in a raised scissor lift painting the cinder block wall in the truck bay or dock area above a closed overhead dock door. The door was not secured. The warehouse manager arrived at work and used the remote control unit in his car to open the overhead door. The railings on the scissor lift were at an elevation that allowed the opening overhead doors to push the scissor lift over into the dock area. The son managed to climb over to a window ledge and was rescued. Employee #1 fell with the lift and sustained fatal injuries.
