Investigation Summary
Employee #1 was removing a part from a revolution mechanical power press. The press began to cycle repeatedly, crushing two of Employee #1's finger tips. What caused the press to cycle without touching the two-hand trip is unknown. However, it was determined that the press did not have control reliability as the air valve could become stuck in the open position causing the press to cycle repeatedly.
