Employee killed in fall from forklift work platform

On August 17, 2001, Employee #1 was using a platform elevated on the forks of a lift truck to install screws into a steel building frame. He fell approximately 18 ft from the platform to the ground, sustaining internal injuries. Employee #1 was transported to the hospital, where he died seven days later. The platform had no guardrails and Employee #1 was using no form of fall protection.
