Employee struck by falling container and killed

Employee #1 was loading an empty vacuum container, which had been picked up by the rear mechanism, and was being pulled forward to fasten to the front chain mechanism. The container rolled off the left side of the trailer and struck Employee #1. He received broken ribs, a broken femur, and broken hips. He was killed as a result of his injuries.
