Employee Injured In Fall

Employee #1 was climbing out of a 12.5-ft by 12.5-ft concrete diversion structure. Employee #1 was using an extension ladder. The 11-ft section of the extension ladder collapsed at the 8-ft level and Employee #1 fell inside the structure to the ground. Employee #1 suffered lower back pain and numbness to his legs. Employee #1 was hospitalized.
