Investigation Summary

At 11:30 a.m. on January 15, 2001, Employee #1 was crushed between a flatbed tractor trailer and a forklift. The forklift was stationary, approximately 3 to 5 feet behind the flatbed tractor trailer where Employee #1 was disconnecting a chain that was wrapped around the forks of the forklift. Employee #1 was facing the forklift while the truck driver was backing the flatbed tractor trailer to maneuver around a pickup truck. The pickup truck was parked on the left hand side in front of the flatbed tractor trailer, and was preventing him from driving straight out and to east side of the shop yard. The driver stated prior to getting in the cab of the tractor trailer, Employee #1 was standing beside the rear of the trailer on the driver's side. When the driver was ready to move the truck, he looked in the rear view mirrors but Employee #1 was not standing there anymore. The driver assumed Employee #1 had gone in to answer the phone or to help a customer. Employee #1 apparently did not notice the tractor trailer backing up and therefore was not able to move out of its path before being crushed to death.
