Investigation Summary
At approximately 5:54 p.m. on April 24, 2001, Employee #1 was in the process of delivering a car. Employee #1 parked the pick up and transporter along the curb on a hill. Employee #1 chocked and blocked the trailer wheels and set the emergency brake on the pick up. Employee #1's son went onto the trailer, got in the car and started to drive the car down the ramp. As the car (Honda Civic) passed over the center trailer wheels, the trailer started to slide down the hill with the car and driver still on the trailer. Employee #1 chased after the pick up and transporter, reached into the driver's window of the pick up just as the vehicles jack knifed. Employee #1 was crushed between the pick up and transporter and was killed.
