Investigation Summary
At approximately 3:30 p.m. on April 13, 2001, Employee #1 was finishing work on top of an 18-foot-high tank which had no guardrails around the top. He stepped from the top of the tank onto a purlin (part of the roof truss system). The purlin gave way under the weight of Employee #1, subsequently sending him to the floor. Employee #1 sustained fractures to his pelvis and left arm, lacerations, and contusions. He was hospitalized for his injuries.
