Employee killed while ridding on a forklift truck

Employee #1 was ridding on the back of a forklift sitting on the propane tank. A load of PVC pipe, measuring 21 inches wide and 21 ft long, was tilted too far back on the forks. The load rolled off the forks and a pipe rolled over the rollover protection on the forklift and struck Employee #1 in the head, killing him.
