Investigation Summary
On December 24, 2000, Employee #1 was filling a propane tank when the bottom of the cylinder ruptured, causing it to become a projectile. The cylinder projectile struck Employee #1 in her head. The impact of the projectile resulted in blunt force trauma to the cephalic (head) region. Emergency medical services were summoned, but she succumbed to her traumatic injuries and died.
