Investigation Summary
At approximately 12:10 a.m. on November 14, 2000, Employee #1 died when a 16,000 pound steel beam fell off of a truck and struck his head. Employee #1 was working as a rigger/helper in partnership with another employee who was operating an overhead bridge crane. The two employees were in the process of loading a flatbed truck with forty foot long pieces of steel tubing. The truck had already been loaded with three stacks of forty foot long by forty wide flange beams. The Model Number of the beam was W8. These beams were placed in stacks four beams high with two stacks on either edge of the trailer and one stack in the middle. Employee #1 and the crane operator were loading the steel tubing onto the truck and placed it in between the stacks of beams. The employees "picked" one piece of tubing by hooking a set of chain slings on either end and placed it on the truck. Employee #1 then jumped upon the bed of the truck to release the chain from his end. As he did this, the crane operator began raising the hoist. Employee #1 then released the chain before the rigging had completely cleared the stack of beams. The hook became caught on a flange of one of the beams, knocking the stack off of the truck. Employee #1 fell to floor, and the 16,000 pound beam landed on him killing him.
