Investigation Summary
Employee #1 was using a New Holland skid-steer loader, Model #553, Serial Number 721875, to move soil from a trench spoil pile to a remote soil storage pile. Employee #1 was found, after a coworker heard him yelling out and waving for his attention, with his lower right leg pinned between the center hydraulic cylinder frame recess and the hydraulic cylinder and with his left leg straddled over the step-in pad and down inside the driver's compartment. It was determined from employee interviews that Employee #1 was reentering the loader while the engine was still running and while the bucket was still in the air. His left foot, when entering, contacted the boom lift control, depressing the toe of the pedal, which is the boom lowering mechanism. By leaning his hand on the seat for balance, Employee #1 had deactivated the interlock system by pressing on the seat switch hydraulic lockout. This allowed the boom to lower before Employee #1 could clear his leg from the path of the boom cylinder. Employee #1 was hospitalized with a fractured leg.
