Investigation Summary
Employee #1 worked for a painting contractor who was contracted to scrape and paint the building walls and epoxy the windowsills. One drop had been completed and the crew was in the process of moving the two-point suspension scaffold over to begin another drop. The scaffold was resting on a rolling scaffold and the plan was to move it atop a structural canopy that was over the building doorway entrance. The height of the canopy was greater than that of the scaffold. The end of the scaffold closest to the canopy was raised mechanically and the plank was set atop the canopy. The wheels of the scaffold snagged on the side of the canopy. One coworker was standing on the canopy attempting to push the scaffold away to clear the wheels. Employee #1, who had been on the scaffold working the hoist motor, stepped on to the canopy to help push the scaffold away from the canopy. Employee #1 was also working the hoist motor at the same time. The scaffold flipped backwards and knocked Employee #1 from the canopy. Employee #1 fell approximately 15 ft to 20 ft to the ground. Employee #1 suffered a fracture and was hospitalized.
