Investigation Summary
At approximately 8:40 a.m. on February 16, 2001, Employee #1 was descending the eight-ft step ladder when it slipped. Employee #1 fell approximately four ft landing on his left arm. The fall appeared to result due to a weight shift to one side of the ladder that caused its legs to move causing Employee #1 to fall. Employee #1 was hospitalized where he was operated on to place screws and plates to hold the fractures secure.
