Employee Killed In Fall From Makeshift Work Platform

Employee #1, a plasterer, was working in the steeple area of a church making repairs to the previous plastering job. He was standing on a makeshift work platform of two planks across the steel railings that enclosed the open space of the steeple. Upon the two planks Employee #1 placed a 4-foot by 8-foot sheet of plywood and set an 8-foot stepladder on the plywood. Employee #1 was working from the ladder when the planks and the plywood work platform dislodged. Employee #1 fell approximately 32 feet to the concrete floor, receiving fatal injuries.
