Investigation Summary
On November 17, 2000, Employee #1 and #2 were installing piping within a 7.5-ft-deep, 28-ft-wide, and 200-ft-long trench without personal protection equipment (e.g., safety line). As they were occupying the excavation site, it collapsed covering both employees with soil. Other employees retrieved the employees from the trench. However, Employee #1 had succumbed to asphyxiation from the lack of aspiration due to the soil's weight. While, Employee #2 was rescue with non-life threatening injuries involving abrasions and contusions to his body.
