Event: 12/06/2000
One employee killed, one injured in fall from scaffold

Employees #1 and #2 were working from a wood pole pump jack scaffold built from 2 by 4 SPF (Spruce-Pine-Fir) lumber. The left pole snapped at a splice approximately 10 ft above the ground, causing both workers to fall approximately 14 ft 3 in. Employee #1 sustained head injuries and was killed; Employee #2 suffered fractures to both legs. Mending plates had not been installed on any of the splices on the poles. In addition, the employees were not trained nor were they being supervised at the time of the accident.
