Employee injured in fall from ladder

On August 23, 2001, Employee #1 of Neth & Son, Inc. was climbing a 20-ft extension ladder to check on the upper section of the roof. While he was climbing, the ladder slid out from under Employee #1, causing him to fall 8 ft, 8 in. to the ground. The employee hit a 30-in. wall in the fall. The ladder was at an angle exceeding one to four and was not secured. The plywood section that the foot of the ladder sat upon was also unsecured. The plywood acted like a sled, easily sliding along the granular roofing material. Employee #1 suffered contusions and abrasions and was transported to the local hospital where he received treatment and was released.
