Employee dies when load of pallets fall on him from truck

On December 7, 2001, Employee #1 was driving a load of wooden pallets in a semi trailer truck. When he arrived he got out of the truck and opened the trailer doors. The pallets fell on him and knocked him to the ground. He was hospitalized and died five days later.
