Employee killed in fall from roof of building

At 2:30 p.m. on March 8, 2001, Employee #1 was installing roofing and insulation without any fall protection. The tin roofing was stored on top of the building roof. A gust of wind picked up a 25 foot long piece of tin roofing and forced it against Employee #1, causing him to step onto roofing insulation. He fell through the insulation, falling approximately 18 feet onto the concrete floor, resulting in fatal injuries.
