Employee crushed when caught in bending machine

At approximately 9:30 p.m. on October 13, 2000, Employee #1 and four coworkers were performing preventive maintenance on a Glasstech lehr. One coworker was the machine operator; (Employee #1 and the other three coworkers were millwrights. The operator was cleaning the area around the machine when the millwrights arrived. One locked out the cold ring side of the machine and went between the shuttle and the die area. Another went to the top of the machine's bender section and took off a large wing nut to lower the mold down onto a carriage that the remaining two millwrights had pushed into place under the mold. One of these workers was looking for a bar to hit the suspension rods of the mold and knock it free while the other--Employee #1--had climbed between the hot ring and the bender area of the lehr. The operator began to lower the mold with the pendant control, which had been placed in the jog position, but it did not move. He went into the control room and quickly switched the button from jog to automatic and back to jog, causing the machine to cycle. The workers had not shut off and locked out the hot ring side and it moved, crushing and killing Employee #1. Jogging the controls was not the normal procedure for when the mold stalled, but the workers had discovered that it was an effective way of resetting the controls.
