Investigation Summary
Employee #1 was assigned to retrieve and place engines and transmissions that were in a warehouse. He was operating a manual pallet jack in the warehouse. He had reportedly been trained on how to use the pallet jack. He was using a Big Joe Lift Truck, Model Number 991305, which was bought used about ten years ago. They did not have the operator's manual for the jack. Employee #1 placed an engine on a shelf that was about 4 ft above the ground. He was moving backwards and pulling the empty pallet jack down an aisle with the forks still elevated when the pallet jack overturned toward him. The jack struck him, which caused him to fall to the concrete floor. The back of his head struck the concrete floor and he died.
