Investigation Summary

A Mack dump truck, owned and operated by Watson Construction Company, was being used to haul dirt from one area of a construction site to another. The driver had an obstructed view to his rear, but the truck was equipped with a reverse signal alarm and lights that were in proper working order. As the driver of the truck drove it in reverse on a road that was not open to the public, Employee #1 walked in its path, unseen by the driver. The employee was struck and run over by the truck, and was pronounced dead at the scene.
