Investigation Summary
On August 12, 2002, Employee #1 was using a John Deere front-end loader, Model Number 444G, to off-load hardwood logs from tractor trailers onto a concrete platform at a sawmill. One side of the platform was between two and three feet above the ground. There was no guardrail or other barrier on the edge of the platform. Employee #1 was backing the loader when it went over the edge of the platform and rolled over onto its side. Employee #1 was ejected from his seat and his head and chest were crushed under the loader's cab. He was killed.
