Investigation Summary
Employee #1, a dredge helper, had completed assisting the dredge operator with the fueling of the dredge when she was asked by the tugboat Captain to check for debris on the tugboat propeller. There was no planning for the diving operation, and no discussion of the safety and health aspects of the dive. She entered the water with no special equipment and while attempting to clear the propeller under the tugboat, she drowned.
