Employee Killed After Falling Through SkyLight

On October 5, 2001, Employee #1, a carpenter, was on a roof of a school gymnasium to remove existing skylights. On the roof, there were four skylights that were to be removed and covered. Later, the covered skylights would be roofed over when the roof of the building was reconstructed. Employee #1 was working on the last of the skylights. He had already removed the skylight and was cutting the wood border around the opening. The skylight opening was 38 inches wide by 38 inches long. His employer did give him any fall protection, but a safety monitor on the roof would occasionally look at him. Employee #1 fell 31 feet through the opening and was killed when his head struck the gymnasium floor.
