Employee killed after falling through skylight

On September 6, 2001, Employee #1 was repairing a leaking skylight on a low-sloped metal roof. Employee #1 used a ladder to get on the roof and fell through the skylight that he was to repair. Employee #1 fell approximately 25-ft onto the concrete floor of the building and died from a blunt force injury to the head.
