Employee Killed by Fall from Roof

On April 30, 2001, Employee #1 and a coworker were assigned to paint the tile roof of an entranceway. The entranceway was 14 ft wide by 14 ft long and the eaves of its roof were 14 ft 8 in. above the ground. The roof had a 4 to 12 pitch. Employee #1 used an extension ladder to gain access to the roof and was using a 3-ft spray wand to paint the roof. The coworker was placing a ladder on the other side of the entranceway when he heard Employee #1 fall. Employee #1 was working on the roof with no fall protection; and he either fell or slipped from the roof to the ground, striking his head on the concrete walkway. Employee #1 was killed.
