Employee #1, a superintendent for the general contractor, died when he was struck by a falling sprinkler pipe. The owner for a steel erection company had returned from a trip to get drinks for his employees who were installing a seam metal roof onto a warehouse addition. The owner went to a Sizzor lift (mobile scaffold) that was located on the east side of the warehouse addition. The lift was used as an elevator to take the employees to the roof at the 39-foot level. The platform of this lift usually remained in the elevated position until someone needed to return to the ground. The owner called several times for the foreman, who was on the roof, to come to the Sizzor lift to lower it, but the foreman never responded. The owner, who had not been trained on the operation or safety of the lift, attempted to lower the lift using the lower controls. The owner stated that he had only opened the panel for the lower controls when Employee #1 arrived at the lift and told the owner that he could lower the lift. Employee #1 was not wearing a hard hat. Employee #1 knelt down to pull the red handle for the manual hydraulic release that was located between the wheels at the front of the lift. The platform of the lift lowered. As the platform lowered, a suspended metal sprinkler pipe fell to the ground and struck Employee #1 in his head killing him. The sprinkler pipe was 24 feet long. The pipe had been suspended from the ceiling structure by using one attachment at its center. However, some believe that it is possible that the lift platform may have struck and dislodged the pipe from its single attachment point.
