Employee killed by fall

On February 17, 2001, Employee #1 fell through a doorway opening. Employee #1 was finishing concrete or attempting to do so, along the edge of a stairway opening on the sixteenth floor when he fell six flights to the tenth floor. Employee #1 was not using any type of conventional fall protection. Employee #1 sustained multiple injuries from the fall and died. Multiple employer worksite.
