Investigation Summary
Employee #1, an operating room nurse, was responsible for loading, running, and unloading of the surgical equipment sterilizers, in the absence of individuals from central supply who usually handle this task. Employee #1 was exposed to some of the contents of the sterilant dispensing container. In improperly washing out the container, Employee #1 inhaled concentrations from the residue of paracetic acid and the proprietary buffers which were the former contents of the cup. Over the following week, Employee #1 was required to be off duty on doctor's orders, and was hospitalized for an overnight stay for treatment and observation for respiratory irritation and distress.
