Investigation Summary
Employee #1 died when he fell from the trailer after being struck on the head by the trailer frame. He was seated on the tongue of the trailer, which was being pulled by a tractor. He fell from the trailer tongue and was struck on the head by the trailer frame. Employee #1 was then pinned under the trailer and pulled approximately five feet before coming to a stop.
