Investigation Summary
Employee #1 was standing on floor trusses on the second floor of a residence under construction ready to secure a steel center microlam beam that was being hoisted by a crane. He was struck in the head by the beam which knocked him over. His foot caught in the floor trusses and he was suspended by his foot until lifted down. Employee #1 died.
