Investigation Summary
At approximately 9:30 a.m. on December 13, 2001, Employee #1 was setting dies in a Scotchman 10-ton single throw rocking beam power press, Model Number 4010C, Serial Number 6870FF197. As the die was lowered to the plate, it shattered and a piece of the die struck the employee in the right eye, penetrating his brain. Emergency medical services were called and the employee was transported to Parkland Memorial Hospital. Life support was removed and Employee #1 was pronounced dead at 10:00 p.m. on December 14, 2001.
