Employee killed after struck by front-end loader bucket

On August 23, 2001, Employee #1 and a coworker from Image Concrete Company were setting concrete blocks in place. Employee #1 was under the elevated bucket to release a chain from a concrete block that he had set in place. He asked for assistance from a coworker. The coworker was not experienced in operating the front-end loader. He unintentionally moved the control handle that would drop the bucket, and this struck a fatal blow to Employee #1.
