Investigation Summary
Employee #1 was in a bucket on the boom of an articulating arm bucket truck. He was actuating the boom to make sure all air pockets were out of the recently repaired hydraulic system. The bucket apparently fell to the ground with the rapid movement, causing Employee #1's head to strike the control panel and throwing him out of the bucket onto the pavement. Employee #1's head injuries proved fatal.
