Employee killed in fall from roof of house

At approximately 9:00 a.m. on October 12, 2000, Employees #1 and #2 were removing shingles from the roof of a split-level house and attached garage. Employee #2 was pulling shingles when he stepped backward off the house roof, fell onto the garage roof, rolled off, and landed, head first, on the driveway. Employee #2 was killed.
