Employee dies when he slips and falls from roof

On October 11, 2000, Employee #1, a roofer, was preparing for work activities, attempted to traverse an intermediate hip roof and lost his balance. He landed on a Westerly facing low sloped roof, slid 14 to 16 ft on the frost covered surface, and fell 28 ft striking an asphalt driveway. Employee #1 died.
