Investigation Summary
At approximately 9:45 p.m. on December 16, 2000, Employee#1, a parking lot attendant, parked a car on the 4th sub-basement level of the parking garage. To return to the parking garage entrance Employee #1 jumped on the moving man-lift and rode it to the street level. When the man-lift reached the street level the employee did not get off of the man-lift. Employee #1 continued to ride it towards the bottom of the floor to the mechanical area. This crushed Employee #1 between the bottom of the mechanical area floor and the platform on the man-lift. Employee #1 was found dead by other employees approximately 15 minutes later.
