Employee Struck by Moving Vehicle

On December 6, 2000, Employee #1 of Carolina Container Company was leaving the facility, walking towards his vehicle in the parking lot. Another employee was exiting the parking lot at that time and struck Employee #1 with his vehicle. Employee #1 was transported to Frye Memorial Hospital by Emergency Medical Personnel and was treated for his injuries and released. Employee #1 was taken back to the parking lot by the plant manager to retrieve his vehicle and drove himself home. Employee #1 returned to work a couple of days after the accident. At approximately 9:00 a.m. on February 22, 2001, Employee #1 passed away at his home. On April 2, 2001, the sister of Employee #1 reported that the death was a possible fatality. The Safety Compliance Officer found no evidence through employee interviews, observations, photographs, sketches, or the inspection, that the accident that occurred on December 6 and the death on February 22 violated any occupational safety and health standards.
