Employee injured when arm caught by conveyor system

On November 16, 2002, Employee #1 became entangle in the conveyor system on the power screener machine. He lifted the door guards and was trying to make some adjustment when his arm was pulled into the rear conveyor roller. The door guard was bent and the mechanism to hold the door in place was missing. The machine was not turned off and locked out before reaching into the ingoing nip point. Employee #1 was transported by helicopter to the hospital.
