Investigation Summary
At approximately 12:30 p.m. on June 29, 2001, Employee #1, age 19, was working on a road resurfacing project when he was struck by the tandem wheels of an asphalt resurfacing/milling machine as it moved along the road. Employee #1's feet, leg, and chest were crushed, and he died 10 days after the accident. This machine was mixing oil with the ground-up asphalt and applying it back to the road surface. A passenger in a vehicle driving by the scene reported the accident, though there were no direct eyewitnesses.
