Investigation Summary
At approximately 10:00 a.m. on July 25, 2000, four employees were installing an 18 inch force main in a trench that was approximately eight feet deep and ten feet wide with no shoring and little or no sloping. The trench collapsed, trapping Employees #1 and #2 in the moving soil and killing them. Employee #3 was trapped and sustained strains to his back.
