Investigation Summary
At approximately 12:30 p.m. on August 8, 2000, Employee #1 was adjacent to a log loader truck whose operator was swinging the truck boom around to grapple a 5 ft 9 in. long, partially cut section of an approximately 8 ft 10 in. long by 4 ft 6 in. diameter log. The loader operator turned the log, trying to break free the partially cut section. As the operator lifted the log, it made a half turn and slipped from the grapple. Employee #1, who was standing sideways between the grapple and the truck bed, was struck by the log. He became pinned against the truck bed, and was forced to the ground by the weight of the log. Employee #1 sustained massive neck and head trauma injuries; he was pronounced dead at the scene.
