Investigation Summary
On October 31, 2001, Employee #1, a resistance welder operator, applied upward pressure on a resistance welding machine, while guiding a file with his opposite hand. The resistance welding machine cycled, broke the file, and crushed his left hand between the closing jaws. The machine power had not been turned off. As a result, Employee #1 lost several fingers. Employee #1 was hospitalized, where his ring finger was reconstructed and pinned, middle finger reattached, and extender tendons reconnected.
