Employee falls from scaffold

Employee #1 working from a height of 11 ft on a walk-through welded steel frame scaffold when a platform plank he was working from gave way. He fell 5 ft to the deck. The scaffold deck had been working from was not placed on the top rails of the welded steel frames but rather supported from bars hooked over a frame piece on the walk through of the steel frames. The bar hooked over one frame piece shifted allowing the scaffold plank to fall. The fall caused injuries to his neck, shoulder, and back.
